What are the hours of operation?Primary Care Monday - Thursday 8am - 6pm (Last appt at 5pm) Friday 8am - 1pm Urgent Care Monday - Friday 8am - 6pm (Last walk-in at 5:45pm)

What is included in my membership?A membership with Heart of Hope Health gives you unlimited access to provider scheduled visits (sick, in-person, or virtual), educational resources, weight management, tailored nutrition support, various procedures ranging from laceration repair to EKG, exclusive supplements, mental health resources, and spiritual support. We believe that individual patient’s needs include more than just physical health and we agree to help you on your wellness journey.

Do you provide care to non-members?Yes, most definitely. You can pay for services individually.

Does Heart of Hope accept health insurance?We do not accept insurance. Removing ourselves from the traditional medical model of health insurance, allows us to treat our patients in the way they want, not based on insurance reimbursement. By removing all the insurance intermediaries, we can decrease health care costs for our patients and reduce a significant workload on our medical staff, thus freeing them to spend more quality time with you. Our membership packages are transparent, simple, and affordable. Using the Direct Patient Care model, our patients will enjoy a different healthcare experience: a personalized care experience with direct access to your provider. If you have insurance, you can still utilize your insurance benefits for referrals. We will forward your insurance information to any specialists we refer to and we will send a copy to our lab partner, so you can apply your insurance benefits to your labwork if you choose to do so. Often times cash prices are cheaper than insurance options, but we will do what you ask us to do. “

What if I want to cancel my membership?“You can cancel your membership before you have fulfilled your 6 month commitment as outlined in the membership agreement, however you will still be responsible for paying your monthly membership dues until 6 months is reached whether or not you use services. Our monthly membership with a commitment of 6 months is much shorter than the standard of 12 months, and allows us to keep our monthly memberships affordable for all. If you choose to go to a different provider, we can fax or give a printed copy of your medical record to you or your new provider if you request. We will print the first 5 pages for free, but additional pages are 0.25 cents per page. Please note that the initial membership enrollment fee is non-refundable, and if you would like to rejoin the practice in the future, another one time enrollment fee will apply.”

Do I still need health insurance if I have a membership with Heart of Hope Health?Although we can assist you with a variety of health and wellness concerns at Heart of Hope Health, sometimes people may require healthcare services that we do not provide. Therefore, we encourage our patients to continue to maintain an insurance policy or cost share option to cover in-person specialist visits, advanced imaging, advanced procedures/ surgeries, and possible hospital services.

Can I use my HSA account to pay for the membership fee, or can it be tax deductible?"Unfortunately, HSA funds cannot be used to pay for care memberships, however TREATMENTS are covered by the HSA funds. If you incur any additional costs during your treatment plan, such as, oxygen tubing, medication administration, lab testing, an x-ray, etc., your HSA or FSA may be applied. Remember to keep your receipts for submission to your plan of choice.

Are there any hidden costs?No. Heart of Hope Health believes honesty, integrity, and transparency are non-negotiable core values. A large amount of our varied services are covered under the membership fee. However, if a service or procedure has an additional charge not covered by membership (PAP smears, labs, ultrasound, injections etc.), you would be notified at the time of service. We would not perform the test unless you approved of the charge. The service, test, or procedure will be invoiced and paid for at the time of service.

How does payment work?You can enroll in your preferred plan of choice in person or via phone. You can choose simple automatic monthly or annual billing via bank transfer, credit card payment, checks made payable to Heart of Hope Health, PLLC, or cash. Any additional treatment costs are reviewed, invoiced, and paid for at the time of service.

How can I help?You can volunteer, donate, and share our practice and website with family and friends.

What if I get sick when I am out of town?You can contact Heart of Hope Health by phone, or telemedicine video chat during business hours. Many conditions can be diagnosed or treated virtually in a very timely manner. We can assist you in getting the medications you need at nearby pharmacies. If you need to be seen for an assessment in-person, we can help you find a resource near your location. With Heart of Hope Health, you are never alone in meeting your health needs.

How do I contact you?For routine questions about primary care services please call 208-500-0601 during our normal business hours; Monday - Thursday 8am -6pm and Friday 8am - 1pm. For questions regarding Urgent Care services please call 208-500-0110 during our normal business hours; Monday - Thursday 8am -6pm and Friday 8am - 1pm. In am emergency call 911. We are not an emergency room. We provide primary and urgent care services only.
